2025-2026 MVR Request Instructions for Public School Bus Drivers
Service Oklahoma has released updated instructions for schools requesting Motor Vehicle Records (MVRs) for school bus drivers. Please review the following guidelines to ensure your requests are submitted correctly:
Important Reminders:

There is no $25 per driver fee for public school MVR requests. Previous confusion about this cost has been clarified.

No major changes this year, except:

A new form is available for individual driver requests (linked below).

Self-addressed, stamped envelopes are no longer required.

Submission Guidelines:

Schools with fewer than 10 drivers: Submit a separate Motor Vehicle Request for Records form for each driver.

Schools with 10 or more drivers: Submit your request on official school letterhead, including:

A list of drivers (see sample letter and format for guidance)

A brief letter requesting the records
